by Nexum HC Consultants PLLC (“Nexum HC”).

1. Program Name & Service Description

Nexum HC Notifications is the official administrative messaging program for
Nexum HC Consultants PLLC. Nexum HC is a clinical mental health practice
providing therapy, counseling, and care coordination services through
in-person and HIPAA-compliant virtual platforms.

2. Emergency & Crisis Disclaimer

NEXUM HC DOES NOT PROVIDE 24/7 CRISIS SERVICES.

SMS, email, and online communication tools are for administrative coordination
only and are not monitored in real time.

If you are experiencing a life-threatening emergency, call 911
or the 988 Suicide & Crisis Lifeline immediately.

Do not use SMS or email for urgent or emergency clinical needs.

3. SMS & Electronic Communications

By providing your mobile phone number and opting in, you provide prior express
consent to receive SMS, MMS, and voice communications from Nexum HC through
authorized service providers.

Consent is collected via website forms, electronic intake documentation, or
documented verbal consent. Consent is not a condition of receiving treatment.

Authorization Requirements

Some services require prior authorization. Services may be paused if not approved.

Eligibility Responsibility

You are responsible for maintaining active insurance coverage.

Financial Responsibility

You are responsible for copays, deductibles, and non-covered services.

Cancellations

Late cancellations or no-shows may incur fees and are not billable to insurance.

Billing Corrections

Nexum HC may correct billing errors and resubmit claims when necessary.

Care Coordination

We may coordinate care with other providers when appropriate and authorized.

Any legal claim must be brought within one (1) year.
