These Terms & Conditions govern your use of services and communications provided
by Nexum HC Consultants PLLC (“Nexum HC”).
Nexum HC Notifications is the official administrative messaging program for
Nexum HC Consultants PLLC. Nexum HC is a clinical mental health practice
providing therapy, counseling, and care coordination services through
in-person and HIPAA-compliant virtual platforms.
NEXUM HC DOES NOT PROVIDE 24/7 CRISIS SERVICES.
SMS, email, and online communication tools are for administrative coordination
only and are not monitored in real time.
If you are experiencing a life-threatening emergency, call 911
or the 988 Suicide & Crisis Lifeline immediately.
Do not use SMS or email for urgent or emergency clinical needs.
By providing your mobile phone number and opting in, you provide prior express
consent to receive SMS, MMS, and voice communications from Nexum HC through
authorized service providers.
Consent is collected via website forms, electronic intake documentation, or
documented verbal consent. Consent is not a condition of receiving treatment.
Message frequency may vary based on your care needs.

protected health information (PHI) could be accessed by unauthorized parties.
If you are consenting to services for a minor, you confirm that you are the
legal parent or guardian with authority to consent.
Communications for minors will be directed to the parent or guardian unless
otherwise authorized.
You are responsible for verifying your insurance coverage and benefits.
Coverage information is not a guarantee of payment. Clients are responsible for
non-covered services.
Some services require prior authorization. Services may be paused if not approved.
You are responsible for maintaining active insurance coverage.
You are responsible for copays, deductibles, and non-covered services.
Late cancellations or no-shows may incur fees and are not billable to insurance.
Nexum HC may correct billing errors and resubmit claims when necessary.
We may coordinate care with other providers when appropriate and authorized.
Telehealth services are provided via secure platforms. By participating, you
acknowledge:
Nexum HC is not liable for indirect or consequential damages arising from use
of communication services or the website.
Any legal claim must be brought within one (1) year.
Nexum HC does not sell personal information for marketing purposes.
Third-party providers may be used for operations and are required to safeguard
your data.
Text messaging opt-in data will not be shared outside of messaging providers.
These Terms are governed by the laws of the State of Illinois.
Nexum HC may update these Terms at any time. Updates will be posted with a
revised effective date.
These Terms are publicly accessible and do not require login to view.
