You can request an amendment to your health information if you believe it is incorrect or incomplete. 

Right to Get a List of Disclosures: 

You may request a list of disclosures made of your health information within a specified time period. 

Right to Request Restrictions: 

You can request restrictions on how we use or disclose your health information. 

Right to Request Confidential Communications: 

You can request confidential communication methods, and we will accommodate reasonable requests. 

Right to be Notified Following a Breach: 

You will be notified if there is a breach of your unsecured health information. 

Right to Choose a Representative: 

You may appoint a representative to exercise your rights concerning your health information. CHANGES TO THIS NOTICE: 

We reserve the right to change our privacy practices. Updated practices will be reflected in a revised Notice of Privacy Practices.
